To complete the Application Package to HUD you will need:
  • Complete HUD Application Form 11701
  • Resumé of officers and stockholders
  • Audited financial statements (1 original, 3 copies)
  • Personal credit report application for all 25% or more stockholders and all officers
  • Business qualification application (credit report)
  • Approved warehouse line of credit (direct lenders only)
  • Errors & Omissions / Fidelity Bond Insurance binder (direct lenders only)
  • Photographs of facility
  • Floor plan of facility (can be hand-drawn)
  • Certification letter that applicant has not been denied a license, etc. (see letter below)
  • Quality Control Plan checklist or your written Quality Control Plan (Note: we can provide a plan if you need one; you must have a written quality control plan to get approved)
  • Copy of fictitious name filing approved (if applicable)
  • Copy of your Corporate license
  • Copy of your Articles of Incorporation
  • Application fee of $1,000 payable to HUD
